I'm also facing click sound along with severe pain. 5 years ago, on the right side of my TMJ, I had undergone arthrocentis treatment. Later 2 years back, doctor suggested me to have knife cut surgery as I was unable to open my mouth not even inch. After undergoing MRI and CT scan, reports were shown that tissue was degenerated and hence doctor did surgery. Now I'm facing difficulty in having pain in left jaw as well. I undergone Arthrocentesis treatment. Still no use... Daily I'm having pain killers and night mouth guard in order to sleep. Pls help me in suggesting in getting relief from unbearable pain...

			Dear sir, I am S.Soundararajan from KARUR in TAMILNADU state in INDIA My younger son S. SIVASANKAR was affected by 'FACIAL CLEFT' since from his birth.(BIRTH DEFECT) He was now 13 years old. He had undergone nearly 5 surgeries from his date of birth. (including external dissstraction) I-REPAIR OF FACIAL CLEFT II-PALATE REPAIR III- EXTERNAL DISSTRACTION IV-MINOR CORRECTION IN DISTRACTION PROCEDURE V-COVERAGE OF DISTRACTION HOLE IN BOTH SIDES OF FACE External disstraction surgery was already done at his age of( nearly) 5 and became a failure one. 1) Now some doctors advise us (my son -13 yrs old) to undergo once? ? again a distraction surgery and a Bone craft surgery and both are necessory for the reconstructive procedure for his face. 2) some doctors advise us (my son 13 yrs old) not to undergo distraction surgery and bone craft at this age. First you should take orthodontic treatments like clip like procedures for nearly up to 5-6 years and then you may undergo distraction surgery and Bone craft procedures. Because the strength of bone is very weak.According to his it was found in test that his Blood Haemoglobin point is : 10.5 points clip on mouth clip was put at the mouth one year before distraction surgery was made and the clip on mouth was kept during the time distraction surgery I was very much confused by the Doctor's several opinions I am confused about "WHAT TYPE OF TREATMENT SHOULD TO DO AS NEXT STEP IN TREATMENT PROCEDURE (WHICH TYPE OF TREATMENT AT WHICH AGE STEP wise should be follwed periodically age wise) I had got several opinions from several Doctors suggesting diffrent types ot treatment order. Now I am request you to suggest a proper and best opinion for this treatment procedure when should which type of treatment should be taken. as a final and best decision choosed for him. AN opinion on the type of treatment which is being recommended for my son. , As a world wide famous medical organization ,please give me just a small advice and it will help me to make my own opinion. or forward this mail to a necessory medical advisory team one or to the Doctor who was specialised in this type of treatments. . At any time I will not surely make liable of you. If there is any welfare trust or charities running to help in this treatment ,please mention it because I was a poor one. 			My 8 year old daughter had an accidental fall from about 30 feet height and suffered multiple facial fractures in the lower jaw, upper lip bone and the both the condyles, she lost her front 4 teeth of the upper jaw as well. She has been operated upon for the fractures of the mandimle but the bilateral condylar fractures have been left without surgery with ORIF treatment. I am attaching her CT scan images. This accident happened on 27th August and the surgery was performed on 30th August. The Wiring was removed by the surgeon on 8th September. The bilateral condylar fractures have been left without surgery. Currently she is able to move her lower jaw a little bit but is not able to open it fully. She is on semi solid diet. We have been told that the child shall be rehabilated by extensive jaw exercise regime over the next 2 years in order to regain the lower jaw movement as well as biting power, and we have also been told that the suregery of the condyles is risky and not suggested in this case. We have also been told that the risks involved with this adopted line of treatment (i.e. ORIF folllowed by exercises) is that the child may never fully regain the full jaw opening or unable to have full jaw bite power. Another major risk that has been informed is that the development of the lower jaw shall be impaired and thus the face can have a disproportionate size where the lower jaw might be left smaller than the upper face. We are very disturbed by these possible risks. Please suggest whether we should go for any surgery in order to avoid these possible outcomes.

			Dear Doctor, I have been experiencing pain in left ear and neck (area just below the ear )for almost a year. ENT doctor asked me to get impacted & carious wisdom tooth removed for relief. I got it done in Feb 2015 along with root canal of lower second molar that was infected badly as well. There was some relief for a few months but I am feeling the same pain again, along with severe jaw stiffness and pain in all the upper left teeth as well. The pain often extends to the neck and shoulders. I have been in absolute agony as a result. Please tell me what I should do. Also on the right side I have a broken first molar which led to supraeruption of upper molar. Currently I am due to undergo an orthodontic process to intrude the upper tooth with mini implants. The bottom first molar has undergone RCT as well. Do I have TMJ or something else. Has the RCT on lower second molar failed? Can you please suggest any expert in Delhi who can help me. I am a teacher & am not able to work due to constant pain. ilI will be grateful if you could please help me. 			I was jumped by gangbangers in Chicago I had surgery the doctor only use closed reduction nothing to suspend the bones. 			Dear Sir, I trust this message finds you in the best of your health. Since,i have gone through many journals on genetics of cleft lip and palate deformity, just to get the glimpse of its formation and passage through generation to generation. Though i got many nice journals where i got to gain much info about the entire process, but still i have many queries to ask... My concern to gain more knowledge are into below asked queries: 1. Whether it is possible to suppress the dominating gene for the formation of cleft lip, if detected during the gestation period, like i found the formation of lip starts taking place during the 4-5 weeks of pregnancy. And can same be done for the palate case, if detected somehow with the help of high end technology. 2. If two clefts patients are getting married, and they would like to know about the probability of the cleft that may or may not pass to their off springs. So, its clear that it may pass or sometimes may not. But, Is it possible to suppress or knock out the responsible genes in them and then they should go ahead with their family planning? just to prevent the chances of cleft in their baby. Would it be be possible for me to get some journals to know much in in-depth knowledge of how consanguinity plays role in giving rise to deformities and other conditions in population. 	There are varied topics related to both ablation and reconstruction. Use of nanotechnology in both diagnosis and drug delivery system is catching up. Alternately in reconstruction use of flap survival techniques using both non invasive monitoring systems as well as invasive monitoring systems .

	To have a specific interest in Oral Oncology is encouraging . However the learning curve is quite steep. You will need to do further training in head and neck surgery from some of the recognized centres in India. There is Kidwai that offers fellowship, so also Narayana Hridyalays. All these centres have too many applicants and too little openings. It would help to join for OMFS in one of the centres that does Head and neck work. Do your homework before you decide.
